Summary and next steps
The post-cataract eyewear benefit is not an automatic two-pair allowance. The sequence of surgery and eyewear delivery matters, as do supplier and billing requirements.
- Tell the supplier about each surgery and any glasses already received.
- Ask for a covered standard option before upgrades.
- Confirm deductible, coinsurance and noncovered items.

Use this checklist before an optical purchase. The post-cataract eyewear exception has conditions; a phrase such as “one pair per eye” does not establish the number of pairs payable in your case.
Check the event and previous delivery
Ask the supplier to confirm that the operation included an intraocular lens and to review any eyewear already supplied. Medicare’s benefit is linked to qualifying surgery, not an ordinary annual glasses allowance.
The sequence matters. CMS specifies one pair after the second operation if both eyes were treated sequentially and no eyewear was received between those procedures. Do not buy a second pair on the assumption that an unused first benefit accumulated.
Ask for the standard option first
Medicare describes the exception as standard-frame glasses or contacts after qualifying surgery. Have the supplier identify the actual covered base option before selecting extras. Ask it to distinguish an upgrade from a medically necessary covered feature.
Write down which items you want and ask for a price next to each one. “Premium package” is not enough information to compare an order. You should be able to see which choices can be removed and what the revised total would be.
Build an estimate from your actual coverage
Original Medicare cost sharing generally includes the applicable remaining deductible and 20% of the approved covered amount. Upgrades and other insurance can change the total. Confirm the supplier’s Medicare participation and whether it accepts assignment before ordering.
A generic calculator cannot establish a payable claim. Ask for an itemized written estimate, what information is still provisional, and how the final charge could differ. Medicare Advantage members should confirm plan-specific arrangements directly.
Keep a short purchase record
- Surgery dates and the relevant prescription.
- Previous post-surgery eyewear and delivery dates.
- Supplier name and the person who explained billing.
- Covered option, optional items and quoted patient share.
- Claim or plan reference and instructions if a problem occurs.
Check the final invoice against the order rather than assuming a denial is caused by one particular code. Request the written reason, ask the supplier to review it and follow the applicable appeal instructions. Do not miss a deadline while waiting for an informal promise to “look into it.”
The surgeon should guide prescription timing and recovery concerns. New pain or worsening sight should be taken to the surgical team promptly, independent of any insurance discussion.

This worksheet does not establish coverage or eligibility. Use the Part B model only after the provider confirms a covered service and accepts assignment. It uses deductible applied + 20% of the remaining approved amount + extra charges. Medigap, Medicaid, other insurance, facility charges, excess charges, and plan-specific rules are not modeled. A plan quote is entered directly rather than applying the Part B formula.
