Summary and next steps

The benefit follows qualifying cataract surgery with an IOL. Do not assume two unused pairs accumulate after two operations; ask the supplier about the actual surgery and eyewear sequence.

  1. Give the supplier both surgery dates and prior eyewear history.
  2. Confirm Medicare participation and assignment before ordering.
  3. Get separate covered and upgrade prices in writing.

Medicare cataract surgery glasses rule

Before ordering glasses, confirm how your surgery history, previous eyewear and chosen supplier fit the post-cataract benefit. “One pair per eye” is an incomplete shortcut and should not be treated as an automatic allowance for two pairs.

Start with the qualifying surgery

Part B has an eyewear exception after cataract surgery that implants an intraocular lens. It covers a pair with standard frames or a set of contacts, subject to the benefit rules. Routine glasses are generally outside this exception.

Tell the optical supplier the surgery dates, which eye was treated and whether glasses or contacts were supplied between operations. Ask the surgeon when a useful prescription can be finalized. A billing article should not decide clinical timing.

Do not assume unused pairs accumulate

CMS explains that when both eyes are operated on sequentially and no glasses or contacts are received between procedures, only one pair is covered after the second operation. Waiting until both operations are complete does not automatically create two unused pairs to claim later.

Describe your actual sequence rather than asking only, “Do I get one pair per eye?” Ask the supplier to explain the rule that applies and to document the expected claim.

Separate the covered item from optional choices

Ask whether the supplier participates in Medicare and accepts assignment for the items. Request a written quote separating the covered option, remaining deductible, coinsurance, upgrade items and any secondary payment.

After the deductible, Original Medicare generally leaves 20% of the approved amount for covered corrective lenses. Extra frame costs and noncovered items can add more. An upgrade recommendation is not itself proof of coverage.

Coordinate another vision plan explicitly

If you have vision insurance or Medicare Advantage, ask the plan and supplier how the actual order will be handled. Do not assume discounts can always be combined or that two plans will pay the same expense.

  • Which payer receives each item?
  • Which items are covered, discounted or fully self-pay?
  • Is the quote based on confirmed eligibility?
  • What happens if the claim is denied?

Keep the quote, prescription, delivery date and any benefit explanation. A clear record can help resolve questions but cannot guarantee reimbursement. If symptoms worsen after surgery, contact the surgical team instead of waiting for a glasses order.

1. Choose the billing situation

This worksheet does not establish coverage or eligibility. Use the Part B model only after the provider confirms a covered service and accepts assignment. It uses deductible applied + 20% of the remaining approved amount + extra charges. Medigap, Medicaid, other insurance, facility charges, excess charges, and plan-specific rules are not modeled. A plan quote is entered directly rather than applying the Part B formula.
